2025 Community Health Assessment

 

The Public Health Accreditation Board defines community health assessment (or CHA) as a comprehensive picture of a community's current health status, factors contributing to higher health risks or poorer health outcomes, and community resources available to improve health. To summarize, it helps us determine the needs of the community through stakeholder and resident feedback.
What are the benefits of a CHA?
  1. Improves organizational and community coordination and collaboration.
  2. Strengthens partnerships within state and local public health systems.
  3. Identifies strengths and weaknesses to address in quality improvement efforts.
  4. Benchmarks for public health practice improvements
  5. Baselines on performance to use in preparing for accreditation.
We're excited to begin this process of creating a Community Health Assessment that centers the Montclair community and provides us with the understanding of the needs of our community that will in turn help us create a Community Health Improvement Plan to follow.
The Community Health Assessment Survey will be released in December 2025 and remain open into mid-January 2026.
